Diabetic shoes, dentures, eye glasses, cell phones



CUMMING, Ga. — Community Services Event at “The Place of Forsyth County”, 2550 The Place Circle, Cumming, Ga.

Resources for Diabetic Shoes, Dentures, Eye Glasses and free cell phones.

This event will be held at The Place on Nov. 7 through Nov. 9 each day from 11 a.m. to noon and 1 p.m. to 4:30 p.m.

Be sure to bring your proof of government assistance and photo identification.

Eligibility Requirements: Note: There is no Forsyth County residency restriction, except for food.

1. Diabetic Shoes - Medicare with Diabetic diagnosis by Doctor. (Co-Pay or Waived if Diabetic & Qualified)

2. Dentures and Eye Glasses - Medicare & Medicaid with Diabetic diagnosis by Doctor. (Free if Diabetic with Medicare & Medicaid)

3. Cell Phones - (Free with Photo ID and Proof of Government Assistance).

Food Stamps, Medicaid, Supplemental Social Security Income (SSI), National School Lunch Program, Federal Public Housing Assistance, Unemployment, LIHEAP (Energy Assistance) Photo ID and Proof of Benefit gets the client a free cell phone and 250 free monthly minutes voice or text with no contract.

Food Pantry is available for Forsyth County residents in need from 11 a.m. to noon and 1 p.m. to 4 p.m.
