The first is for us to come and provide transitional care for him at your home. We can provide care from one hour to 24/7 or live-in. This initial extra care that we can provide will help with the transition and help to determine his current needs to develop a long-term care plan. After this initial transition period, we can discuss further care at home or other living options for him. Q: I live in an Assisted Living here in Johns Creek and I need some extra help.
